Service Solutions Facilitator – Halton Region

Hours: Monday to Friday, 8:30 am to 4:30 pm

CWSDS is looking to hire a Service Solutions Facilitator (Complex Support Coordinator), on a full time basis. The position will report to the Manager, Community Networks of Specialized Care, and will be based out of our main centre in Oakville, ON.
Responsibilities include, but are not limited to,
- Achieving positive results for people by through direct service delivery which provides complex support coordination in accordance with the CNSC model
- Cross-sector coordination support (e.g. assessment, clinical planning, housing, planning tables) to enhance existing resources and services and to ensure efficient use of said resources
- Facilitating the acquisition of required supports and services (e.g. specialized services, residential etc.)
- Liaising with different sector partners, including those in the health care system, mental health and justice
- Assisting with system navigation, including local and regional service solutions, urgent response and other planning tables
- Referring and facilitating access to and from specialized resources, including at the local, regional and provincial levels. This would include clinical supports, psychiatric support, psychological support and specialized accommodations
- Monitoring active and inactive files (e.g. have the goals been met or are other service coordination supports in place)
- Developing appropriate strategies for people who may be resistant to services and who live at risk in the community
- Participating on community committees, working groups or other tables as requested
- Supporting the Vision, Purpose and Vales of CWSDS and the CNSC
- Complying with CWSDS policies and procedures
- Completing statistical information and other required data entry in a timely manner
- Other duties as assigned
QUALIFICATIONS:
- Completion of a formal university degree in Social Services at the undergraduate or post graduate levels. A combination of various work and education experience will also be considered
- A minimum of 5 years of experience in applying a person-centred and recovery oriented service philosophy to the provision of community support services for individuals with a developmental disability or complex and multiple needs
- Demonstrated facilitation, interviewing, planning and service coordination skills and ability to promote and build consensus and resolution
- Strong ability to network, and develop plans for individuals with complex and multiple needs
- Excellent written, verbal and interpersonal communication skills
- Thorough understanding of the bio-psychosocial factors contributing to developmental disability, mental illness and the life situation of affected individuals
- Positive attitude and ability to work collaboratively with formal and informal cross-sector community services/resources
- Proven skills in showing good judgement and ability to problem solve and manage challenging situations
- Well-developed risk assessment and advocacy skills
- Ability to work independently with minimal direct supervision and apply judgement on an independent basis
- A satisfactory Police Records Check (Vulnerable Sector Screening)
- A valid G Ontario driver’s license and vehicle to travel throughout the Region
