We are looking to hire a Dual Diagnosis Justice Case Manager on a full time, permanent basis. This position will work at the Central West Network of Specialized Care, reporting to the Manager, Community Networks of Specialized Care.

LOCATION: This position will be responsible for a caseload within Halton Region, based out of our Oakville office. Some travel across regions will be required.

 

RESPONSIBILITIES:

  • Working collaboratively alongside police, other court support workers and discharge planners to implement bail release and court diversion plans for people with developmental disabilities and/or dual diagnosis
  • Capacity building and support to community services providers in developmental services, mental health and justice services about the unique and often complex needs of people with a dual diagnosis.
  • Providing consultation and education to the Police/OPP, the criminal justice sector, mental health and developmental services, regarding both individual and system specific issues to ensure that the needs of people with a developmental disability and/or dual diagnosis are being addressed.
  • Assisting individuals, families and service providers to understand the vulnerabilities faced by this population.
  • Partnership and planning with mental health court support workers, local correctional and custody facilities for appropriate discharge planning.
  • Coordinating a continuum of services based on individual choice and need to maintain the individuals in the community with adequate cross sector supports
  • Assisting, where appropriate, criminal justice officials in securing access to needed forensic assessment or other specialized assessments.
  • Engaging individuals and families in addressing immediate and long-term needs (e.g. shelter, food, medical attention) while linkages to longer term supports are being organized
  • Providing assistance, support, and education to the person’s formal and informal support system.
  • Linking people to preferred services and resources, as well as desired services within the mental health and developmental sectors
  • Developing appropriate strategies for people who may be resistant to services and who live at risk in the community
  • Enhancing the understanding of individuals, family members and service providers of their legal rights in the areas of housing, finances, work/education, and medical/psychiatric services (e.g. Mental Health Act).
  • Advocating on behalf of people with a developmental disability/dual diagnosis for the protection of personal and legal rights, and for access to needed services, resources, and supports to live successfully in the community
  • Other duties as assigned

 

QUALIFICATIONS:

  • Completion of a formal University Degree in Social Services or related field. A combination of various work and education experience will also be considered
  • Progressive training and development in organization systems and/or community development
  • A minimum of 5 years’ experience in applying a person-centered and recovery oriented service philosophy to the provision of community support services for individuals with a developmental disability or dual diagnosis.
  • Sound working knowledge of the criminal justice system, the Mental Health Act including involuntary admission process, the assessment provisions of the Criminal Code, the Developmental Services Act, related health care legislation and forensic issues
  • Thorough understanding of the bio-psychosocial factors contributing to mental illness, developmental disability, and the life situation of individuals with a dual diagnosis
  • Ability to work collaboratively with formal and informal community services/resources within the criminal justice, mental health and developmental sectors
  • Ability to work independently with minimal direct supervision and apply good judgement on an independent basis.
  • Well-developed risk assessment and advocacy skills
  • Strong organizational, communication, interpersonal and problem-solving skills.
  • A satisfactory Police Records Check (Vulnerable Sector Screening)
  • A valid Ontario’s driver’s license and vehicle to travel through the Region
