Supportive Living Homes

We provide Supportive Living to people at eleven homes in the communities of Oakville, Burlington, Mississauga, Halton Hills and Georgetown.

Services Included in Supported Living

Our supportive living homes offer person-directed services to improve the quality of life of the people we support and help them reach their full potential. This includes specialized services such as behavioural services, clinical services, and day services.

Specialized Geriatric Supports

Two of our Supportive Living programs are dedicated to the needs of older adults with intellectual and developmental disabilities. We are committed to prioritizing a person’s dignity through their changing health and wellness needs, allowing them to age in place with minimal disruption.

How to Access Services:

CWSDS Supportive Living Services are provided to persons aged 18 years or older based on a referral from Developmental Services Ontario (DSO). To determine eligibility for services and complete an assessment prior to a referral, please contact Developmental Services Ontario (DSO), 1-888-941-1121.

Placements are made based on availability, suitability, and are prioritized by need.
