Continuum of Care

Our continuum of care ensures people with developmental disabilities and their families have access to the right services, at the right time, at every stage of life. Our programs are tailored to each individual’s needs, blending clinical expertise, person-centered planning, and community collaboration.

Our Approach

Our Services

Through our interdisciplinary team, we deliver specialized short-term assessment and treatment to persons living with dual diagnosis, enabling them to transition successfully back into their home and community.

We work with healthcare, social services, and community partners across the Central West Region to improve coordination, access, and quality of care for people with intellectual and developmental disabilities, dual diagnoses, and complex needs.

We develop person-directed plans that focus on each person’s interests, strengths, and aspirations, helping them engage meaningfully in daily activities and community life.

We administer the provincially funded Passport Program, providing financial support to people with developmental disabilities, empowering them to participate more fully in their community and achieve their personal goals. We operate Supportive Living homes in Oakville, Burlington, Mississauga, Halton Hills, and Georgetown,  to provide enhanced support, clinical care, and opportunities for personal growth.

We provide comprehensive multidisciplinary assessment, consultation, treatment planning, and support for people with co-occurring intellectual and mental health challenges in the Central West Region, empowering them lead healthier and more independent lives.

We offer service coordination, respite care, and community-based programs tailored to meet the needs of people with developmental disabilities and their care providers in the Halton Region.

We provide short-term relief for families and caregivers by providing an enriching environment for people living with intellectual and developmental disabilities.
