Central West Network of Specialized Care

We work with cross-sector community partners in the Central West Region with the goal of improving the coordination, access and quality of services for people who have an intellectual and developmental disability, dual diagnosis and high support and complex care needs.

Central West Passport Program

We administer the provincially-funded PassportONE program that provides funding for people who have an intellectual and developmental disability to more fully participate in their community.

Day Services

Through Supportive Living Services, we develop person-directed plans and help people achieve their goals based on their interests and strengths.

Dual Diagnosis Services

We provide consultation, assessment, treatment planning, support and education to people with a dual diagnosis and their care providers who live in the Central West Region.

Halton Support Services

We provide service coordination, respite and other community-based services to people and their care providers who live in the Halton Region.

Respite Services

We provide non-emergency respite care for families of people who have developmental disabilities and complex needs.

Short Term Assessment and Treatment Environment (STATE)

Through Supportive Living Services and our multi-disciplinary team, we provide short-term assessment and treatment for people with a dual diagnosis to help integrate them back into their home and community.

Supportive Living Services

We provide Supportive Living to people at eleven homes in the communities of Oakville, Burlington, Mississauga, Halton Hills and Georgetown, as well as through three additional programs that offer specialized supports.
